Hm... Matsumoto would call people by given names, Renji and Ikkaku and the thugs in general probably would too. Though on that note, did you have Iba in there? Because Ikkaku and the others usually call him "Tetsu" instead of his full given name of Tetsuzaemon...
...Rukia would probably call most people by surnames unless she's particularly familiar with them. Less politeness so much as habit, I think.
Ah, and on that note, people like Ishida would call Sado that instead of Chad...
Yeah, I remembered Iba. And I think he should still have a bit of a thing for Matsumoto, not that he can do anything about it while they're in seperate gangs.

I think only Ichigo's gonna call him Chad... And maybe Mizuiro and Keigo...
